Read all the way to the bottom of this information sheet before you use the online program to fill out your application. The computer program helps you fill in an application for free legal help from Southeast Louisiana Legal Services, or "SLLS" for short.
THIS FORM IS NOT FOR LEGAL EMERGENCIES!
If you have an emergency or a deadline, do not use the online form. If your problem needs any kind of action within the next 15 days, do not use this form! Call or go to your local legal services office during business hours. (Note: the online program may say 30 days rather than 15).
YOU MUST SAVE YOUR COMPLETED FORM AND SEND IT BY E-MAIL TO selegal@slls.org. Use only selegal@slls.org. Do not send the application to any other e-mail address, even if you think it is an SLLS address.
IF YOU USE A PUBLIC COMPUTER YOU MUST BRING A DISK OR POCKET FLASH DRIVE TO SAVE YOUR APPLICATION. DO NOT SAVE YOUR APPLICATION ON A PUBLIC COMPUTER!
This is only step 1 to get legal help. Sending in the form does not make SLLS your lawyer.
If you do not know how to use a computer well enough to fill out and to e-mail this form, stop! Do not use this form. Call or go to your nearest SLLS office.
If you are filling this application out for someone else, you must have their permission to do so. After this application is received, staff will have to speak with the applicant in order to complete this process.
You must use the online computer program to create your form and you must use e-mail to send in the form. Use your own e-mail address (if you have one) and your own phone number.
DO NOT MAIL OR FAX YOUR FORM. SLLS WILL NOT PROCESS MAILED AND FAXED FORMS.
The computer program creates a WORD document. If you use another program, this form may not work for you, and SLLS may not be able to open and read your form.
Make sure you have all of the pages. The pages are numbered and the words “END OF FORM” appear at the end of the last page. Answer every question that applies to you. Check your answers. Leaving out items asked for can slow down your application!
Print out the form and keep it before you send it by e-mail.
Attach the file with your completed form to an e-mail addressed to selegal@slls.org.
When you e-mail your form type “Application for Help” in the subject line of your e-mail. The label in the subject line helps SLLS see that the e-mail is not spam or an advertisement.
Step 2 is: Once SLLS gets your form someone from SLLS will call you to get more information about your legal problem. Therefore, a reliable telephone number where we can reach you is important.
If you do not hear from SLLS within 10 calendar days of e-mailing your form to selegal@slls.org, call or go to your nearest SLLS office during business hours. Tell SLLS that you sent an application by e-mail and when you sent it. Explain that you e-mailed your form to apply for legal help but SLLS did not call you.
Step 3: your case will be reviewed by staff after you have been interviewed. At the review a decision will be made as to whether we can help you. Unless you have been told that we will be able to take your case, SLLS IS NOT YOUR LAWYER.
Important: if you know that Southeast Louisiana Legal Services (SLLS) is handling a case against you, do not use this online form.
SLLS' responsibilities as lawyers are as follows: SLLS will not give this information to the attorney in our office who is handling the case against you. SLLS will continue to handle the case against you.
SLLS does not handle criminal matters. Do not use this form if you have a criminal issue or case.
SLLS helps people who live in its service area. Do not use this form unless you live in one of the 22 parishes on the list.

If you know who is on the other side of your issue or case (the "opponent" or "adverse party"), please include that information in the online form. This information is needed even if you are looking for help with an uncontested divorce.
Note: if you experience "pop ups" while using the online program, you may need to unblock your pop ups to complete the program and to get your cocmputer to give you your form.
